Janet Duni, RN, BSN, MPA, CCM, is the Director of Care Coordination at Vanguard Medical Group. Vanguard is an independent, 8-site primary care organization with subspecialties in sports medicine and gastroenterology, serving 60,000 patients in northern New Jersey.
Janet is nurse executive with expertise in creating population health strategies and driving patient engagement through value-based initiatives with CMS, ACOs, and commercial payer programs.
Over the past seven years, Janet has led a team of nurses and support staff in a robust and innovative care coordination program which has directly supported patient care efficacy and the financial goals of the practice. Janet developed and implemented a medical assistant (MA) in-service continuing education series, which was selected as one of five winning programs in the 2015 AMA-MGMA Practice Innovation Challenge.
Janet earned her Bachelor of Science in Nursing from Rutgers University, and a Master of Public Administration in Health Policy from Columbia University. Janet also holds a Certification for Case Management (CCM) from the Commission for Case Management.
