The Diabetes Digital Health Learning Network (DDHLN) is a collaborative effort between Welldoc and
clinicians working in diabetes care to:
- Explore technology-enabled diabetes population health
- Create opportunities to discuss experiences with using evidence-based digital technology in clinical practice
- Leverage patient-generated health data to optimize self-management and treatment plans to improve outcomes

The Montana DDHLN: A Collaboration with the Montana DHHS Diabetes Program, the Montana Coordinating Body–Association Diabetes Care and Education Specialists (formerly known as the Montana Diabetes Educators Network – CB of AADE) and Welldoc. The Montana DDHLN is composed of
clinicians and health coaches from hospitals, diabetes programs, tribal clinics and community programs throughout Montana.
